Please complete the form below to register with Integrative Laboratory Services. This service is available to Canadian regulated health care practitioners, including Naturopathic Doctors, Nurse Practitioners, and Medical Doctors.

Your account will be activated within 24 business hours after registration.

Practitioner details

This password will allow you to Log In to your ICL account to update address and credit card information as needed. It also provides access to our test menu and fee schedule. Please note, for privacy purposes, a separate log in is required to access results from the ICL Results Portal.
